From time to time, the researchers at the Mary Kay O’Connor Process Safety Center analyze safety issues which may be of importance to the industry as well as the public-at-large. While these analyses are done within the overall mission and scope of the Center, it must be understood that these topics or issues are not selected in any organized manner. These issues may have been brought to our attention by individuals or organizations and after analysis and study we have come to certain conclusions. In some cases these issues may have been derived from the knowledge base and experience of the Center personnel. Comments about these Safety Alerts may be submitted to the Center Director.
The Center provides these Safety Alerts with the sincere hope that they will lead to an increase of safety awareness and decrease of avoidable accidents. However, the Center encourages all users of this information to make sure that the information is applicable to their specific circumstances.
